Tirzepatide (CAS 2023788-19-2), is a first-in-class dualGIP receptor (GIPR) and GLP-1 receptor (GLP-1R) agonist - a 39-amino acid acylated synthetic peptide of the dual incretin mimetic chemical class. The sequence has a new Aib-containing GIP/GLP-1 hybrid backbone with a C20 fatty diacid chain (eicosanedioic acid) linked to it by a Lys26-γGlu-AEEA linker. The GIPR agonist part of tirzepatide works in addition to GLP-1R agonism to have anabolic and insulinotropic effects. These include increased insulin secretion, decreased glucagon, improved insulin sensitivity, and direct effects on adipose tissue through GIPR. The Phase 3 SURPASS trials showed that people lost up to 22.5% of their weight and their HbA1c levels dropped by up to 2.47%.
