Retatrutide (CAS 2381089-83-2) is a new drug from Eli Lilly and Company that works on three receptors: the GIP receptor (GIPR), the GLP-1 receptor (GLP-1R), and the glucagon receptor (GCGR). It is the first GIP/GLP-1/glucagon triple agonist peptide for obesity and type 2 diabetes. Retatrutide, is a modified 30-residue acylated peptide with a C20 fatty diacid (eicosanedioic acid) modification at a central lysine. This modification allows for once-weekly subcutaneous dosing through albumin binding. The Phase 2 results showed that people lost up to 24.2% of their weight over 48 weeks, which was the most weight loss ever reported for a therapeutic in a Phase 2 study at the time. The triple agonism is thought to work together to help people lose weight and improve their metabolism by using GIP-mediated effects on fat tissue, GLP-1-mediated effects on satiety, and glucagon-mediated effects on energy expenditure and lipolysis.
